Tirzepatide’s Effects on Muscle and Lean Mass: A Review

Tirzepatide, a dual glucose-dependent insulinotropic polypeptide and glucagon-like peptide-1 receptor agonist, has garnered attention for its efficacy in promoting weight loss among individuals with overweight and obesity. This narrative review aims to evaluate the current evidence surrounding tirzepatide’s impact on body composition, specifically focusing on changes in lean mass and its proxy measures.

The review is based on a structured literature search that identified ten studies, which included randomized controlled trials, observational studies, and modeling analyses. These studies encompassed diverse populations, some with type 2 diabetes mellitus and others without. Across all studies, tirzepatide consistently produced large, dose-dependent reductions in body weight. Notably, these reductions were primarily attributed to fat mass, although decreases in lean mass, fat-free mass, or skeletal muscle mass were observed in all studies.

The proportion of total weight loss attributable to lean mass generally ranged from 10% to 30%. This range aligns with expected physiological responses to weight loss, suggesting that while tirzepatide is effective for fat loss, it may also lead to some reduction in lean mass. However, the review highlights substantial heterogeneity in study design, population characteristics, and body composition assessment methods, which limits the ability to draw definitive conclusions.

Most studies relied on indirect measures such as bioelectrical impedance analysis, with limited use of dual-energy X-ray absorptiometry, which is considered a more accurate method for assessing body composition. Importantly, the review notes that functional outcomes were not assessed, leaving the clinical significance of the observed reductions in lean mass uncertain.

Overall, tirzepatide appears to promote preferential fat loss, but the concurrent loss of lean mass raises concerns. The authors suggest that strategies such as resistance training and adequate protein intake may help mitigate potential adverse effects on muscle mass during treatment. However, further research is needed to clarify the long-term implications of tirzepatide on body composition and functional outcomes.

What this study does not show

This review does not establish a clear clinical significance of lean mass reductions associated with tirzepatide treatment, nor does it assess functional outcomes related to these changes.
